Draft - Data Entry Form for CMMAP Educational Resources Database

We have asked the scientists (and other members of the Education Team) to share resources (images, animations, PowerPoint presentations, etc.) relevant to educational efforts. Our intent is to construct a database to house these materials, and web-based interfaces to that database. We will need a data entry form and a search form.

This page is a draft mock-up of the interface to the data entry form, created by Randy Russell at UCAR. This is not functional, and doesn't reflect much effort towards making the interface aesthetically pleasing. It is an attempt to include the appropriate interface elements.

I haven't included details concerning how folks will actually upload their items, since I don't know how that will be implemented.

Note: when the submitter clicks the "Submit" button, if the "contributor same as creator" check box is not checked, a second form that asks for the name, email, and affiliation of the creator(s) should appear. That form should include an option to list multiple creators.
